Hospice palliative care supports individuals to achieve the best possible quality of life by providing compassionate care during serious illness or at end-of-life through physical, emotional, social and spiritual support that also complements professional medical care for people to live with comfort, dignity and peace.

Over the past 30 years, Hospice Toronto has provided and advanced compassionate care and leadership through innovative programs and services in volunteer and community-based, and in-home supports and group activities, including:

·         In-home, Care Team* and children’s support for individuals requiring care at home, and their caregivers and loved ones.

·         Complementary therapies provided by registered professionals in Expressive Arts, Healing and Therapeutic Touch, legacy work, massage therapy, Reiki and Reflexology to help manage pain and stress.

·         Creating Caring Communities to address the varied and complex health care needs of new Canadians, seniors and people living marginally in some of Toronto’s diverse communities.

·         Expressive Arts – group and arts-based activities for adults, children, caregivers and friends to share their grief and loss to enable them to begin the healing process or cope with issues relating to illness and aging.

·         Young Carers Program group-based, peer support for children and youth in a significant caregiving role for a loved one at home.

·         Bereavement support for caregivers, and family and friends through one-on-one and group support, while also providing linkages to information and referrals to other community services.

As Toronto’s first home hospice palliative care program, Hospice Toronto has become a recognized leader in providing high-quality, hospice care and support for people living with serious illness, and their caregivers and loved ones, at no charge. Since 1988, Hospice Toronto has served over 11,000 individuals (directly impacting the lives of over 40,000 family members and friends).

In celebrating our 30th anniversary this year, Hospice Toronto is evolving to meet the needs of our rapidly aging population, and people living longer with chronic illness.  Our 30th Anniversary is a time to celebrate our many accomplishments, remember those we have served, honour our volunteers and supporters, and to prepare for the future.

*  A group of family, friends and community members coming together to provide additional support for the loved one with serious illness, and their caregiver to enhance overall care.
